Services for Current Consumers

Accepting applications for new consumers
We will still be taking applications for new consumers, determining eligibility and completing Individual Plans of Employment for new consumers.
Step 1: Get an an application, sign and return release forms
Staff members can drop off packets at your house or you can come pick forms up at the office. Call for an appointment to pick up paperwork or to have it delivered.
Our office will be available 8-4:30 Monday through Friday FOR DROP OFF AND PICK UP OF PAPERWORK , BY APPOINTMENT ONLY. If forms are dropped off at the office or picked up at your home, you can sign the release forms and return those at the same time. It only takes a few minutes to complete the release forms. Keep the application for later!
Applications can also be mailed or emailed to you or downloaded from our website. You can drop off your signed release forms, we can pick the forms up, or you can mail back to us at:

Step 2 Determine eligibility - this can take up to 60 days, so apply now!
To be eligible for vocational rehabilitation services you must:
- Have a documented physical, mental, or sensory impairment
- Have a substantial impediment to employment
- Require VR services to maintain employment or enrollment in school
- Be a member of a federally recognized tribe
- Live on or near the Spirit Lake reservation.
Step 3 Assessment and Preparing for Your Employment Plan
Together with your counselor, you wil explore job goals and decide the best way to attain them.
You'll also discuss available jobs. Remember that application you received in Step 1? We'll start filling that out.
Step 4 Individualized Plan for Employment
Your counselor will make an appointment when you can go over your employment goal and needed services. Until the tribal administration re-opens buildings to the public, the IPE meeting will be conducted over the phone. Remember that application you received in Step 1? We'll go over the forms together.

Step 5 Follow your plan and receive services
Once your IPE is written you can receive services that are necessary and reasonable to get and keep a job. Services can include; counseling, help with job applications, gas to get to interviews, tools or clothing required for a job and more. Step 6 Employment!
Even after you are employed, if you need short-term assistance to keep your job, we are here for you!
